Radio button yes no going to different pages

I feel silly for asking, but I can’t figure out how to apply a radio button selection to a next button.
I have a Yes radio button and a No radio button I want to be able on click of a ‘submit’ button be able to send the user to a different page depending on which option they clicked.

tried to work it out but really need an rp file if anyone has this with the above working
